Sculptra Timeline: What to Expect Week by Week
Why Sculptra looks like it wore off at week two, when collagen starts to show, how many sessions it takes and when we judge the final result.

After Sculptra, the face looks fuller for a few days, but that is mostly water and it fades by about week two, when many people think it has worn off. New collagen then builds gradually. Most people have about three sessions at least three weeks apart, and we judge the full result around three months after the last one. In the FDA cheek wrinkle trial, 94 percent were still rated improved at two years.

If your face looks at week two as if your first Sculptra session wore off, that is expected. In this guide I walk through the Sculptra timeline week by week: what you see on the day, why the early fullness fades, when collagen starts to show, and when we judge the final result.
The Sculptra timeline at a glance
Sculptra results arrive in two waves: a short-lived fullness from the water it is mixed with, then a slow build of your own collagen over the following months. This table shows the typical pattern for a facial course.

Day 0 to week 2: why it looks like nothing happened
The fullness you see in the mirror on treatment day is mostly water, and it is meant to fade. Sculptra is a powder mixed with sterile water before injection. A 2024 review in the Journal of Cosmetic Dermatology by Avelar and colleagues notes that this immediate volume lasts several days, until the water is absorbed. By the end of the second week most people look much as they did before treatment.
This is expected, and it is also why a careful injector does not try to make you look “finished” on the day. The FDA label warns against overfilling and says hollows “are expected to gradually improve over several weeks” as the treatment takes effect.
The first five days at home
After treatment, the area needs to be massaged for five minutes, five times a day, for five days, often called the 5-5-5 rule. The label also asks you to limit sun, UV lamps and extreme temperatures until any swelling and redness have gone. In an Arizona summer that means shade, a hat and no hot yoga or sauna for those first few days. Bruising usually fades within a week or so; makeup can go on once the injection points have closed.
Weeks 3 to 12: building the series
Sculptra is planned as a short series because each session builds on the last. Galderma states that results develop over an average of three treatments spaced at least three weeks apart. In the FDA trials patients had up to four sessions, three or four weeks apart, and the original smile line trial averaged 3.2 sessions.
Collagen takes time to form. In a biopsy study by Goldberg and colleagues, summarized in the same 2024 review, type I collagen in the treated skin was 65.5 percent higher at three months. The first visible change usually shows around week six, and it continues gradually from there.
Three months after your last session: judging the result
We assess the full result about 12 weeks after your final session, not after the first one. If you are halfway through a series, it is too early to judge. In Galderma’s cheek wrinkle trial, which led to the April 2023 FDA approval, 96 percent of patients were rated improved at the three-month assessment. At that point you can compare photos with your injector and decide if you are done or if one more session would help a particular area.
Late lumps: Small nodules under the skin, when they happen, tend to appear late. In the original FDA smile line trial, nodules showed up at a median of 160 days. One to two years and beyond: how long the result lasts
The FDA states that the effects of poly-L-lactic acid may last up to two years. In the cheek wrinkle trial, 94 percent of patients were still rated improved at one year and at two years. In the label’s post-approval study of 867 patients, investigators rated 96.4 percent as improved at year two and 91.2 percent at year three. Your own collagen keeps aging at its normal pace, so a common approach is a maintenance session once the result softens, rather than a full new course.
How the Sculptra timeline compares with filler and Botox
Sculptra is the slowest of the three by design, and that is the trade-off for a gradual, longer-lasting change.

Why does my face look the same two weeks after Sculptra?
The immediate fullness was mostly water, which is absorbed within days. Collagen building has started but is not yet visible. This is expected.
How long does swelling last after Sculptra?
Swelling, tenderness and any bruising usually settle within a few days to a week. Limit sun and heat until they have gone, as the FDA label advises.
