Hyaluronidase is an enzyme that breaks down hyaluronic acid. Your body already produces it naturally; the injectable form is a concentrated version. Placed into an area of filler, it breaks the gel apart within hours, and your body clears the fragments over the following days.
This is a large part of why hyaluronic acid is the most widely used filler: very little else in aesthetics can be reversed. If a result doesn't feel right, whether it's too full, in the wrong plane, migrated, asymmetric or not what you pictured, you don't have to wait a year for it to fade.
Hyaluronidase works on hyaluronic acid only. Sculptra, Radiesse, permanent fillers such as PMMA, silicone and fat transfer don't respond to it. Identifying what you have is the first step of every consultation, and if it isn't hyaluronic acid, we'll talk through your options before any treatment.

Which product, roughly when it was placed, how much and in what plane. If you aren't sure, we assess by feel and appearance. This is also where we confirm it is hyaluronic acid, since hyaluronidase only works on hyaluronic acid.

We review your allergy history
Hyaluronidase is derived from several sources, and allergic reaction, while uncommon, is possible. We take a full allergy history and can patch test beforehand whenever there's reason for concern.

The enzyme is placed into the filler
Injected directly into the areas holding product. Dosing reflects the volume and how long the filler has been in place; older, more integrated filler generally needs more.

Hyaluronidase breaks the bonds holding the gel together within hours. Most of the visible change happens in the first 24 to 48 hours, which is remarkably fast for an aesthetic treatment.

Two things happen at once. Hyaluronidase temporarily breaks down some of your own natural hyaluronic acid alongside the filler, and that recovers within a day or two. Tissue that has been stretched by filler for months or years also needs time to retract, so for a while the area can look flat, hollow or lax. This is temporary and completely normal, and we'll walk you through it in advance. It's also why we recommend waiting before any rebuild: placing filler before the tissue has returned to baseline can bring back the original concern.

Hyaluronidase is also the emergency treatment for vascular occlusion, when filler enters or compresses a blood vessel. It's a rare but serious complication of any filler treatment, and managing it well depends on recognizing it quickly and having hyaluronidase ready on hand. We keep it on site at every appointment. Will hyaluronidase work on what you have?
It's helpful to confirm this before you book. Hyaluronidase dissolves hyaluronic acid and nothing else. If you had Sculptra, Radiesse or a permanent filler, it won't have an effect, which is why identifying your filler is the first step of every consultation.

How to prep & what to expect after
Helpful, but not required
- Bring any product names, dates or records if you have them. If you have none, come anyway — we assess by feel, appearance and timeline.
- Tell us about any allergies, particularly to bee stings, or any previous reaction to hyaluronidase. We can patch test where there is reason for concern.
- If medically appropriate and cleared by your physician, stop blood thinners, NSAIDs, fish oil and vitamin E for 48 hours.
- Allow at least three weeks before any event. The area often looks flatter than expected for the first week or two.
- Tell us if you have a cold sore history and lips are being treated — we may prescribe antiviral cover.
- Reschedule if you have an active infection in the area.
- Let us know if you are pregnant, trying to conceive or breastfeeding.
The first week
- Expect some immediate swelling from the injection itself. This is separate from the filler and settles within a few days.
- Ice intermittently for the first 24 hours and sleep with your head elevated for two nights.
- Most of the dissolving happens in the first 24 to 48 hours. The difference is usually obvious by the second morning.
- The area may look flat or hollow for a week or two while stretched tissue retracts. This is temporary and expected.
- No strenuous exercise, sauna or hot tub for 48 hours.
- Avoid alcohol for 24 hours.
- Please wait at least two to four weeks before any rebuild, so the tissue can return to baseline and the original concern doesn't recur.
- Call us for spreading redness, heat, hives, wheezing, swelling of the tongue or throat, or any difficulty breathing. Allergic reaction to hyaluronidase is uncommon but needs immediate attention.

Can I have filler again after dissolving?+
Yes, though we ask you to wait at least two to four weeks so the tissue has fully returned to baseline. Rebuilding too soon can bring back the original concern. It's also entirely optional, and many patients dissolve and stop.
Safety
Is dissolving filler safe?+
Generally yes. The main risk is allergic reaction to hyaluronidase, which is uncommon but possible — we take a full allergy history and can patch test where indicated. Swelling and bruising at the injection site are expected.
Does dissolving hurt?+
Comparable to having filler placed, and usually briefer. Topical numbing is available. Expect immediate swelling from the injection itself, which settles within a few days.
