What Sculptra is

A collagen biostimulator,
for gradual, natural change

Sculptra is poly-L-lactic acid, a biocompatible material used in dissolvable surgical sutures for decades. Placed deep beneath the skin, it adds very little volume on its own. Instead, it prompts your fibroblasts to lay down new type I collagen over the following months. The PLLA is fully absorbed, and what remains is tissue your own body built.

Hyaluronic acid filler works the other way around: visible volume the same afternoon, lasting a year or so. Sculptra shows little on the day, builds steadily to a natural result by month three, and holds for up to two years.

Sculptra is a planned course. Most patients have two to three sessions spaced four to six weeks apart, and we assess the result around twelve weeks after the last one. We'll map your full course together at your consultation, so you know what to expect at every stage.

How Sculptra works

Why Sculptra results
build gradually

Sculptra's mechanism is what makes results look so natural, and why a little patience is part of the treatment. Each stage plays a part.

Reconstitution and hydration

Sculptra arrives as a freeze-dried powder. It is reconstituted with sterile water and given time to hydrate well before use. Dilution volume and hydration time directly affect how evenly it disperses, and how likely nodules are.

Deep placement

Injected supraperiosteally or into deep subcutaneous fat, with a cannula or needle depending on the area, in a fanning pattern. Depth is central to safety, since superficial PLLA is what forms visible nodules.

Initial fullness that settles

You'll leave looking slightly fuller from the fluid it was reconstituted in. This settles within a couple of days and you return to your starting point, which is completely expected. We'll remind you beforehand so it's never a surprise.

Fibroblasts respond

Over the following weeks your fibroblasts recognize the PLLA particles and begin producing new type I collagen around them: your own tissue, built by your own cells.

PLLA absorbs, collagen remains

The particles are fully metabolized into carbon dioxide and water. What remains is the collagen scaffold, which is why the result outlasts the product and holds for up to two years.

Reconstitution

Sculptra is reconstituted and given time to hydrate fully. We take our time with this step because it has the greatest influence on your result and on nodule risk.

Numbing

Topical anesthetic is applied, and lidocaine is usually added to the reconstitution itself, so the treatment is well tolerated.

Injection

Deep placement in a fanning pattern across the treatment area, cannula or needle depending on the zone. Twenty to thirty minutes for a facial course.

Massage — the 5-5-5 rule

You massage the area for five minutes, five times a day, for five days. This routine disperses the particles evenly and is your main protection against nodules. We'll show you exactly how before you leave.

and how we prevent them

The complication associated with Sculptra is nodules, small firm lumps under the skin. They're linked to three factors: injecting too superficially, inadequate dilution or hydration time, and skipping the post-treatment massage. All three are controllable. We dilute generously, allow the product to hydrate fully, place it deep, and avoid the lips and tear troughs, where tissue is thin and mobile. Your part is the 5-5-5 massage, which makes a real difference. Most nodules that do form are small and resolve on their own, and those that don't can be treated.

Hyaluronic acid filler can be dissolved within days if needed. Sculptra can't: once the collagen has been built, it stays until your body remodels it over a year or two. For that reason we're conservative with vial count on a first course, and we make sure gradual, diffuse change is what you're looking for rather than defined structure. Adding a vial at your twelve-week review is always easier than having placed one too many at the start.

Sculptra and filler are often compared, yet each does something different beautifully. Sculptra rebuilds a broad collagen foundation gradually and cannot be reversed. Filler adds precise structure right away and can be dissolved. If you'd like a defined jaw for an event in three weeks, filler is the better choice. How to prep & the 5-5-5 rule

Start about a week ahead

Understand the timeline before you book: you'll look unchanged at two weeks. If you'd like a result for an event, start six months ahead, or we can talk about filler instead.
If medically appropriate and cleared by your physician, stop blood thinners, NSAIDs, fish oil, vitamin E and alcohol for 48 hours.
Tell us about any autoimmune or connective tissue condition, and any history of keloid or hypertrophic scarring.
Reschedule if you have an active skin infection, or dental work within two weeks either side.
Plan for the massage: five minutes, five times a day, for five days. Choose a week when that fits comfortably.
Let us know if you are pregnant, trying to conceive or breastfeeding. We'll wait until the timing is right.
