Lip Flip vs Lip Filler: How to Choose

A lip flip changes how much lip shows; filler changes how much lip there is. How each works, how long each lasts, side effects, cost at KORA and how to choose.

If you only read one thing

A lip flip uses a few units of Botox at the upper lip border so the lip rolls slightly outward and shows more; it adds no volume and is an off-label use. Lip filler is hyaluronic acid gel that adds volume and shape. A flip is cheaper and lasts weeks to a few months; filler costs more, lasts about 6 to 12 months and can be dissolved. The two are often combined.

Lip flip vs lip filler is a common question before lip treatment, and the two treatments work in opposite ways. In this guide I explain what each one does, how long each lasts, what the days after look like, what each costs at KORA, and how to decide which one, or both, suits your lips.

A lip flip changes how much lip shows; filler changes how much lip there is

A lip flip relaxes the muscle at the edge of the upper lip so it rolls slightly outward, while lip filler adds volume inside the lip. Neither is a smaller version of the other.

A lip flip uses a few units of Botox placed along the upper lip border, into the orbicularis oris, the ring of muscle around the mouth. Relaxing the edge of that muscle lets the upper lip turn out a little, so more of the pink shows, especially when you smile. Nothing is added, so the lip is not bigger.

Lip filler is a hyaluronic acid gel injected into the lip. It adds volume, so it can make the lips fuller, define the border and Cupid’s bow, and balance the upper and lower lip. Several fillers are FDA-approved for lip augmentation, including Juvederm Volbella XC and Restylane Kysse.

Off-label use: Botox Cosmetic is FDA-approved for frown lines, crow’s feet, forehead lines and platysma (neck) bands, according to its prescribing information. The lip flip is an off-label use. It is common and well described in the medical literature, and off-label use is explained before treatment.

Lip flip vs lip filler side by side

The main differences are what changes, how long it lasts, and how the days after feel.

What the research says about each

The lip flip has a small body of research and lip filler has large approval trials, so the two have very different levels of evidence.

Lip flip

A 2025 systematic review in Archives of Dermatological Research (Pitchford, Desrosiers and Tolkachjov) pulled together the published studies on the lip flip. Typical doses were 4 to 6 units in total, results were often seen within the first week, and in some cases the effect lasted 3 to 4 months. The most common side effects were functional: trouble drinking through a straw, whistling, spitting or saying sounds like “p” and “b”. Most of these resolved within two to four weeks. The authors concluded that the lip flip has high satisfaction and a favorable safety profile, but that larger, controlled studies are still needed.

In practice, a sensible expectation is about 6 to 8 weeks from a lip flip, which is shorter than Botox usually lasts in the forehead or around the eyes (my Botox timeline post covers those). The dose is small and the lip moves constantly.

Lip filler

Lip filler has FDA approval trials behind it. In the Volbella XC lip study, 62% of patients still had at least a one-point improvement in lip fullness at one year. In the Restylane Kysse trial, 78% still felt their lips looked better at 12 months. The trade-off is more after-effects: in the Volbella study, 93% of patients recorded swelling and 89% bruising in their 30-day diaries, most of it gone within two weeks. My post on how long lip filler lasts goes into the longevity data.

Which one suits your lips

The right choice depends on whether your lip lacks volume or simply hides when you smile.

  • Your upper lip disappears when you smileIf the lip has reasonable volume at rest but rolls inward when you smile, a lip flip addresses that directly.
  • Your lips look thin even at restA flip cannot add volume that isn’t there. Filler is the treatment that changes it.
  • You want a defined border or Cupid’s bowShape and definition come from filler, placed precisely along the border.
  • You want to try a change firstA lip flip is a lower-commitment way to see how you feel about more upper lip show before choosing filler.
  • You smile with a lot of gumA lip flip is sometimes combined with Botox to the muscles that lift the lip; a 2026 case report in Cureus (Arora and Arora) describes this approach. See my post on gummy smile correction.
  • You play a wind instrument or sing professionallyTell us. A flip can briefly affect fine lip control, so filler may suit you better.

Can you get a lip flip and filler together?

Yes, a lip flip and lip filler can be done at the same appointment, and the combination often means you need less filler. The flip adds upper lip show; the filler adds shape and volume where it’s needed. The amounts are planned at your consultation, based on your lip shape at rest and when you smile.

Aftercare differs. After a lip flip, follow the same rules as Botox elsewhere; my post on what not to do after Botox covers them. After filler, expect a few days of swelling and follow my lip filler aftercare guide. First-time lip filler patients come back for a two-week review at no charge.

Frequently asked questions

Is a lip flip the same as Botox?

Yes. A lip flip uses a small dose of Botox placed along the upper lip border. It is an off-label use, since Botox Cosmetic is FDA-approved for other areas.

Does a lip flip make your lips bigger?

No. It rolls the upper lip slightly outward so more of it shows. No volume is added.

How long does a lip flip last compared with filler?

A lip flip usually lasts weeks to a few months; a 2025 review reported up to 3 to 4 months in some cases. Lip filler usually lasts about 6 to 12 months.

What are the side effects of a lip flip?

The most common are brief trouble drinking through a straw, whistling or saying some sounds. In the 2025 review, most of these resolved within two to four weeks.

Can I get a lip flip and lip filler at the same time?

Yes. It is a common combination and often means less filler is needed.

Which is cheaper, a lip flip or lip filler?

The lip flip. Lip flip at KORA Aesthetics: $100 Flat Fee. Lip filler: Starting at $650+.

How do I know which one I need?

If your upper lip disappears when you smile but looks fine at rest, a flip may be enough. If your lips are thin at rest, filler is what adds volume.
